streamline ICMP implementation

  • generate ICMP identification based on fibril counter
  • use fibril-local ICMP sequence numbers
  • use hash table (instead of char map) for ICMP replies
  • simplify locking
  • simplify ICMP communication
  • Property mode set to 100644
